In Australia, podiatry is classified as an allied health profession, and is practised by individuals licensed by their representative State Boards of Podiatry. There are seven registration boards and six teaching centres, with two levels of awards — unclassified bachelors degree and honours level. In Australia there exist 2 levels of professional accreditation and professional privilege: Podiatrist and Podiatric Surgeon.

Australian Podiatrists must register with the Podiatry Board of Australia. The Podiatry Board of Australia is responsible for regulation and recognition of Podiatrists  and Podiatric Surgeons, and assessing foreign trained registrants

Australian podiatrists complete an undergraduate degree ranging from 3 to 4 years of education. The first 2 years of this program are generally focused on various biomedical science subjects including anatomy, medical chemistry, biochemistry, physiology and patient psychology, similar to the medical curriculum. The following two years will then be spent focusing on podiatry specific areas such as podiatric biomechanics and human gait, podiatric orthopaedics or the non-surgical management of foot abnormalities, pharmacology, general medicine, general pathology, local and general anaesthesia, and surgical techniques such as partial and total nail avulsions, wound debridement, and other cutaneous and electrosurgical procedures.

Australian podiatric surgeons are specialist podiatrists with further training in advanced medicine, advanced pharmacology, and training in foot surgery. Podiatrists wishing to pursue specialisation in podiatric surgery must meet the requirements for Fellowship with the Australasian College of Podiatric Surgeons. They first complete a degree of 4 years, which includes 2 years of didactic study and 2 years of clinical experience. Following this, a masters degree must be completed with focus on biomechanics, medicine, surgery, general surgery, advanced pharmacology, advanced medical imaging and clinical pathology. They then qualify for the status of Registrar with the Australasian College of Podiatric Surgeons. Following surgical training with a podiatric surgeon (3–5 years), rotations within other medical and surgeons’ disciplines, overseas clinical rotations, and passing oral and written exams, Registrars may qualify for Fellowship status.[6] Fellows are then given Commonwealth accreditation under the Health Insurance Act to be recognised as providers of professional attention, for the purposes of health insurance rebates.

It is also practiced in other countries such as Canada, Ireland, the United Kingdom, Malta, Cyprus, South Africa, Singapore, Australia and New Zealand. In many English-speaking countries, the older title of “chiropodist” may still be used by some clinicians but is gradually falling out of use. In many non-English-speaking countries of Europe, the title used instead of podiatrist may be “podologist” or “Podólogo”. The level and scope of practice may vary in these countries as compared to the US.

The professional care of feet was in existence in ancient Egypt as evidenced by bas-relief carvings at the entrance to Ankmahor’s tomb dating from about 2400 BC where work on hands and feet is depicted.

Corns and calluses were described by Hippocrates who recognised the need to physically reduce hard skin, followed by removal of the cause. He invented skin scrapers for this purpose and these were the original scalpels. Aulus Cornelius Celsus, a Roman scientist and philosopher, was probably responsible for giving corns their name. Later Paul of Aegina (AD 615-690) defined a corn as “a white circular body like the head of a nail, forming in all parts of the body, but more especially on the soles of the feet and the toes. It may be removed in the course of some time by paring away the prominent part of it constantly with a scalpel or rubbing it down with pumice. The same thing can be done with a callus.”

Until the turn of the 20th century, chiropodists—now known as podiatrists—were separate from organized medicine. They were independently licensed physicians who treated the feet, ankles and related leg structures. Lewis Durlacher was one of the first people to recognize the need for a protected profession. He tried to establish the first association of practitioners in 1854, although it would take another century to come to pass.

There are records of the King of France employing a personal podiatrist, as did Napoleon. In the United States, President Abraham Lincoln suffered greatly with his feet and chose a chiropodist named Isachar Zacharie, who not only cared for the president’s feet, but also was sent by President Lincoln on confidential missions to confer with leaders of the Confederacy during the U.S. Civil War.

The initial society of chiropodists, now known as podiatrists, was established in—and still operates in—New York in 1895 as NYSPMA, with the first school opening in 1911. One year later the British established a society at the London Foot Hospital and a school was added in 1919. In Australia professional associations appeared from 1924 onwards. The first American journal appeared in 1907, followed in 1912 by a UK journal. In 1939, the Australians introduced a training centre as well as a professional journal. The number of chiropodists increased markedly after the Great War then again after World War II.

Increased numbers of ex-soldiers needing to be gainfully employed gave chiropody a boost and led to the need for registration in all English speaking countries. The study of the foot (i.e. podology), brought greater knowledge to the practice of foot care or podiatry.

Ancient Greek scholars Hippocrates and Aristotle wrote about dentistry, including the eruption pattern of teeth, treating decayed teeth and gum disease, extracting teeth with forceps, and using wires to stabilize loose teeth and fractured jaws. Some say the first use of dental appliances or bridges comes from the Etruscans from as early as 700 BC.Further research suggested that 3000 B.C. In ancient Egypt, Hesi-Re is the first named “dentist” (greatest of the teeth). The Egyptians bind replacement teeth together with gold wire. Roman medical writer Cornelius Celsus wrote extensively of oral diseases as well as dental treatments such as narcotic-containing emollients and astringents.

Initially, dental extractions have been used to treat a variety of illnesses. During the Middle Ages and throughout the 19th century, dentistry was not a profession in itself, and often dental procedures were performed by barbers or general physicians. Barbers usually limited their practice to extracting teeth which alleviated pain and associated chronic tooth infection. Instruments used for dental extractions date back several centuries. In the 14th century, Guy de Chauliac invented the dental pelican (resembling a pelican’s beak) which was used up until the late 18th century. The pelican was replaced by the dental key which, in turn, was replaced by modern forceps in the 20th century.

The first book focused solely on dentistry was the “Artzney Buchlein” in 1530, and the first dental textbook written in English was called “Operator for the Teeth” by Charles Allen in 1685. It happened between 1650 and 1800 that the science of today dentistry developed. It is said that the 17th century French physician Pierre Fauchard started dentistry science as we know it today, and he has been named “the father of modern dentistry”. Among many of his developments were the extensive use of dental prosthesis, the introduction of dental fillings as a treatment for dental caries and the statement that sugar derivate acids such as tartaric acid are responsible for dental decay.

There has been a problem of quackery in the history of dentistry, and accusations of quackery among some dental practitioners persist today.

The first dental school, Baltimore College of Dental Surgery, opened in Baltimore, Maryland, USA in 1840. Philadelphia Dental College started in 1863 and is the second in the United States. In 1907 Temple University accepted a bid to incorporate the school.

Studies showed that dentists graduated from various countries, or even from different dental schools in one country, may have different clinical decisions for the same clinical condition. For example, dentists graduated from Israeli dental schools may recommend more often for the removal of asymptomatic impacted third molar (wisdom teeth) than dentists graduated from Latin American or Eastern European dental schools.

In the United Kingdom of Great Britain and Ireland, the 1878 British Dentists Act and 1879 Dentists Register limited the title of “dentist” and “dental surgeon” to qualified and registered practitioners. Nevertheless, others could legally describe themselves as “dental experts” or “dental consultants”. The practice of dentistry in the United Kingdom became fully regulated with the 1921 Dentists Act, which required the registration of anyone practicing dentistry. The British Dental Association, formed in 1880 with Sir John Tomes as president, played a major role in prosecuting dentists practising illegally.

In many countries, a dentist is a healthcare professional qualified to practice dentistry after graduating with a degree of either Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD). This is equivalent to the Bachelor of Dental Surgery/Baccalaureus Dentalis Chirurgiae (BDS, BDent, BChD, BDSc) that is awarded in the UK and British Commonwealth countries. In most western countries, to become a qualified dentist one must usually complete at least 4 years of postgraduate study; within the European Union the education has to be at least 5 years. Dentists usually complete between 5 to 8 years of post-secondary education before practising. Though not mandatory, many dentists choose to complete an internship or residency focusing on specific aspects of dental care after they have received their dental degree.

Dentistry, which is a part of stomatology, is the branch of medicine that is involved in the evaluation, diagnosis, prevention, and surgical or non-surgical treatment of diseases, disorders and conditions of the oral cavity, maxillofacial area and the adjacent and associated structures and their impact on the human body. Dentistry is widely considered necessary for complete overall health. Those who practice dentistry are known as dentists. The dentist’s supporting team aides in providing oral health services, which includes dental assistants, dental hygienists, dental technicians, and dental therapists.

Dentistry commonly encompasses very important practices related to the oral cavity. Oral diseases are major public health problems due to their high incidence and prevalence across the globe with the disadvantaged affected more than other socio-economic groups.

Although today dental practice centres around prevention, many treatments or interventions are still needed. The most of dental treatments are carried out to prevent or treat the two most common oral diseases which are dental caries (tooth decay) and periodontal disease (gum disease or pyorrhea). Usual treatments involve the restoration of teeth as a treatment for dental caries (fillings), extraction or surgical removal of teeth which cannot be restored, scaling of teeth to treat periodontal problems and endodontic root canal treatment to treat abscessed teeth.

All dentists train for around 4 or 5 years at University and qualify as a ‘dental surgeon’. By nature of their general training they can carry out the majority of dental treatments such as restorative (fillings, crowns, bridges), prosthetic (dentures), endodontic (root canal) therapy,periodontal (gum) therapy, and exodontia (extraction of teeth), as well as performing examinations, radiographs (x-rays) and diagnosis. Dentists can also prescribe certain medications such as antibiotics, fluorides, and sedatives but they are not able to prescribe the full range that physicians can.

Dentists need to take additional qualifications or training to carry out more complex treatments such as sedation, oral and maxillofacial surgery, and implants. Whilst the majority of oral diseases are unique and self limiting, some can indicate poor general health,tumours,blood dyscrasias and abnormalities including genetic problems.

Most countries have some method of officially recognizing specialist qualifications in all branches of medicine, including internal medicine. Sometimes, this aims to promote public safety by restricting the use of hazardous treatments. Other reasons for regulating specialists may include standardization of recognition for hospital employment and restriction on which practitioners are entitled to receive higher insurance payments for specialist services.

The issue of medical errors, drug abuse, and other issues in physician professional behavior received significant attention across the world, particularly following a critical 2000 report[ which “arguably launched” the patient-safety movement. In the U.S., as of 2006 there were few organizations which systematically monitored performance. In the U.S. only the Department of Veterans Affairs randomly drug tests, in contrast to drug testing practices for other professions which have a major impact on public welfare. Licensing boards at the U.S. state level depend upon continuing education to maintain competence. In Europe, as of 2009 the health systems are governed according to various national laws, and can also vary according to regional differences similar to the United States.

After graduation, medical practitioners often undertake further training in a particular field, to become a medical specialist. In North America, this is often referred to as residency training; in Commonwealth countries, such trainees are often called registrars.

This further training typically takes from three to six years, but can be longer depending on specialty and jurisdiction. Primary care is increasingly recognized as a specialty, and residency programmes in this field are becoming common. A medical practitioner who completes specialist training in internal medicine (or in one of its sub-specialties) is an internist, or a physician in the older, narrower sense.

In some jurisdictions, specialty training is begun immediately following completion of entry-level training, or even before. In other jurisdictions, junior medical doctors must undertake generalist (un-streamed) training for one or more years before commencing specialization. Hence, depending on jurisdiction, a specialist physician (internist) often does not achieve recognition as a specialist until twelve or more years after commencing basic medical training—five to eight years at university to obtain a basic medical qualification, and up to another nine years to become a specialist.

In Western culture and over recent centuries, common Western medicine has become increasingly based on scientific reductionism and materialism. This style of medicine is now dominant throughout the industrialized world, and is often termed Biomedicine by medical anthropologists. Biomedicine “formulates the human body and disease in a culturally distinctive pattern”, and is a world view learnt by medical students. Within this tradition, the medical model is a term for the complete “set of procedures in which all doctors are trained” (R. D. Laing, 1972), including mental attitudes. A particularly clear expression of this world view, currently dominant among conventional physicians, is evidence-based medicine. Within conventional Western medicine, most physicians still pay heed to their traditions.

The critical sense and sceptical attitude of the Hippocratic traditions laid the foundations of today medicine on broad lines, and we owe to it: first, the emancipation of medicine from the shackles of priestcraft and of caste; secondly, the conception of medicine as an art based on accurate observation, and as a science, an integral part of the science of man and of nature; thirdly, the high moral ideals, expressed in that most “memorable of human documents” (Gomperz), the Hippocratic oath; and fourthly, the conception and realization of medicine as the profession of a cultivated gentleman.

In this Western tradition, physicians are considered to be members of a learned profession, and enjoy high social status, often combined with expectations of a high and stable income and job security. Nevertheles, medical practitioners often work long and inflexible hours, with shifts at unsociable times. Their high status is partly from their extensive training requirements, and also because of their occupation’s special ethical and legal duties. The term traditionally used by physicians to describe a person seeking their help is the word patient (although one who visits a physician for a routine check-up may also be so described). This word patient is an ancient reminder of medical duty, as it originally meant ‘one who suffers’. The English noun comes from the Latin word patiens, the present participle of the deponent verb, patior, meaning ‘I am suffering,’ and akin to the Greek verb πάσχειν (= paskhein, to suffer) and its cognate noun πάθος (= pathos).

Physicians in the narrow sense are usually members or fellows of professional organizations, such as the American College of Physicians or the Royal College of Physicians in the United Kingdom, and such hard-won membership is itself a mark of status.

While modern biomedicine has distanced itself from its ancient roots in religion and magic, many forms of traditional medicine and complementary and alternative medicine (CAM) continue to espouse vitalism in various guises: ‘As long as life had its own secret properties, it was possible to have sciences and medicines based on those properties’ (Grossinger 1980).  The US National Center for Complementary and Alternative Medicine (NCCAM) classifies CAM therapies into five categories or domains, including: alternative medical systems, or complete systems of therapy and practice;mind-body interventions, or techniques designed to facilitate the mind’s effect on bodily functions and symptoms;biologically based systems, including herbalism; and manipulative and body-based methods, such as chiropractic and massage therapy.

In considering these support-traditions that differ from biomedicine, medical anthropologists emphasize that all ways of thinking about health and disease have a significant cultural content, including conventional western medicine.

Homeopathy is a popular complementary trend of medicine. Homeopathy is included in national system of medicine in many countries such as India and neighbouring countries. The practitioners of homeopathic medicine in these countries are generally referred to as homeopathic doctor, homeopath, homeopathic physician on par with conventional medical physicians.

Doctors called osteopaths are recognized as physicians in 48 countries, particularly in the United States, where they are no longer called osteopaths, but osteopathic physicians or just physicians, and where they have unlimited practicing rights in all specialties and subspecialties of medicine. In the USA, osteopathic medical schools (DO) have a curriculum almost identical to allopathic (MD) schools with the exception of osteopathic manipulative medicine, which focuses on extra instruction in the musculoskeletal system. Worldwide, there are variations in the DO degree; osteopathic education includes teaching manipulative medicine. In the U.S.A. the American Podiatric Medical Associaiton (APMA) defines podiatrists as physicians and surgeons that fall under the department of surgery in hospitals.

In India and laerge number of other countries, BHMS (homeopathic) and BAMS (ayurvedic) degree holders like MBBS (allopathic) holders, are treated like a registered medical practitioner. They have the right to practice medicine of their systems. They are referred to as physicians. They have to undergo further specialization post-graduate degree to become a specialist in a particular field of medicine/surgery.

Nurse practitioners (NPs) are not the same as physicians; the American College of Nurse Practitioners do not describe themselves this way. They are classified as advance practice registered nurses/clinicians, and are also known as mid-level (healthcare) practitioners in US government regulations. Nurse practitioners may perform work similar to that of physicians, especially within the realm of primary care, but use advanced nursing models instead of medical models. The scope of practice for a Nurse Practitioner in the United States is defined by individual state boards of registration in nursing, as opposed to state boards of registration in medicine. Physician Assistants are also classified as midlevel advance practice clinicians, have a similar scope of practice as nurse practitoners, and are regulated by state boards of registration in medicine.

The term physician comes from the Ancient Greek word φύσις (physis) and its derived adjective physikos, meaning “nature” and “natural”. From this, amongst other derivatives came the Vulgar Latin physicus, meaning a medical practitioner. After the Norman Conquest, the term entered Middle English, via Old French fisicien, as early as 1100. Initially, physician meant a practitioner of physic (pronounced with a hard C). This archaic noun had entered Middle English by 1300 (via Old French fisique). Physic meant the craft or science of treatment with drugs or medications (as opposed to surgery), and was later used both as a verb and also to describe the medications themselves.

In English, there have been a lot of synonyms for physician, both old and new, with some semantic variation. The noun phrase medical practitioner is perhaps the most widely understood and neutral synonym. Medical practitioner is lengthy but inclusive: it covers both medical specialists and general practitioners (family physicians, family practitioners), and historically would include physicians (in the narrow sense), surgeons and apothecaries. In England, apothecaries historically included people who now would be called general practitioners and pharmacists.

In the USA and Canada, the term physician commonly describes all those holding the degrees of Doctor of Medicine (MD) and Doctor of Osteopathic Medicine (DO). Within North America, the title physician, in this broad sense, also describes the holders of medical degrees from other countries that are equivalent to the North American Doctor of Medicine degrees; typical examples of such degrees from non-North American countries are MB BChir, BM BCh, MB BCh, MB ChB, MBBS, BM, M.B.B.S. etc.. In the US, only those graduating from faculties listed in the WHO Directory of Medical Schools are able to apply for medical licensure in the relevant US jurisdiction, via the ECFMG.

The American Medical Association, established in 1847, currently uses physician in this broad sense to describe all its members. But the American College of Physicians, established in 1915, does not: its title uses physician in an older, narrower sense, as discussed.

Physician is still widely used in its older, more narrow sense, mostly outside North America. In this usage, a physician is a specialist in internal medicine or one of its many sub-specialties (especially as opposed to a specialist in surgery). This initial meaning of physician conveys a sense of expertise in treatment by drugs or medications, rather than by the procedures of surgeons.

This older usage is at least six hundred years old in English: physicians and surgeons were once members of separate professions, and traditionally were rivals. The Shorter Oxford English Dictionary, third edition, gives a Middle English quotation making this contrast, from as early as 1400:

Henry VIII granted a charter to the London Royal College of Physicians in 1518. It was not until 1540 that he granted the Company of Barber/Surgeons (ancestor of the Royal College of Surgeons) its separate charter. In the same year, the English monarch established the Regius Professorship of Physic at the University of Cambridge. Newer universities would probably describe such an academic as a professor of internal medicine. So, in the 16th century, physic meant roughly what internal medicine does now.

Nowaday, a specialist physician in this older, narrower sense would probably be described in the United States as an internist. Another term, hospitalist, was introduced in 1996, to describe US specialists in internal medicine who work largely or exclusively in hospitals. Such ‘hospitalists’ now make up about 19% of all US general internists, who are often called general physicians in Commonwealth countries.

The older, more narrow usage of physician as an internist is usual in the United Kingdom and other Commonwealth countries (such as Australia, Bangladesh, India, New Zealand, Pakistan, South Africa, Sri Lanka, Zimbabwe), as well as in places as diverse as Brazil, Hong Kong, Indonesia, Japan, Ireland, and Taiwan. In such places, the more general English terms doctor or medical practitioner are prevalent, describing any practitioner of medicine (whom an American would likely call a physician, in the newer, broad sense). In Commonwealth countries, specialist pediatricians and geriatricians are also described as specialist physicians who have sub-specialized by age of patient rather than by organ system.

Both the role of the doctor of medicine and the meaning of the term itself vary significantly around the world, but as generally understood, the ethics of medicine require that physicians show consideration, compassion and benevolence for their patients.

The term doctor can be confused with holders of other academic doctorates. Doctor  means teacher in Latin and is an agent noun derived from the verb docere. A cognate expression occurs in French as docteur médecin, a direct equivalent of medical doctor or doctor of medicine, and usually found as its contraction, médecin (doctor, physician).

The Greek term iatrós, means doctor or healer, is often translated as physician. It is not preserved directly in English, but occurs in such formations as psychiatrist (translates from Greek as healer of the soul), podiatrist (foot healer), and iatrogenic disease (a disease caused by medical treatment). In Latin, the term medicus meant much what physician or doctor does now.

The ancient Romans also had the word archiater, for court physician. Archiater derives from the ancient Greek term meant chief healer. By contraction, this title has given modern German its word for physician: Arzt.

Leech and leechcraft are archaic English terms respectively for doctor and medicine. The Old English word for “physician”, læċe, which came from Old High Germanand and Old Irish, lives on as the modern English word leech, as these particular creatures were formerly much used by the medical profession. Cognate forms for leech exist in Scandinavian languages: in modern Swedish as läkare, in Danish as læge, in modern Norwegian as lege (bokmål) or lækjar (nynorsk), and in Finnish as lääkäri. These Scandinavian words still translate as doctor or physician rather than as a blood-sucking parasite.
